Consultation Service AgreementPlease read the following service contract and check the box under each section.Please enable JavaScript in your browser to complete this form.Rover Rehab agrees to provide a behavior consultation and animal training session to the Client, conducted virtually via Zoom. Rover Rehab's trainer will gather information from the Client on training and behavior information relevant to the Client’s goals and begin coaching the Client on how to train and work with their animal(s) on specific training or behavior issues. If applicable, ongoing training and behavior modification after the initial consultation will be covered under a separate service agreement. Rover Rehab will make every reasonable effort to help the Client achieve training and behavior modification goals but makes no guarantee of the animal’s performance or behavior as a result of providing animal training services. *I agreeThe Client agrees to pay Rover Rehab the Virtual Consultation Fee of $175 as indicated at time of booking. This fee is due upon booking to reserve the consultation time slot. If the Client cancels the consultation with less than 24 hours' notice or is a "no show" (i.e. has not entered the virtual meeting room within 10 minutes of the scheduled appointment start time), a cancellation fee of one half of the full consultation fee may be applied. *I agreeThe Client agrees to complete a Training Inquiry Form that will be emailed to them upon confirmation of their appointment. This form will be completed no later than 48 hours prior to their scheduled appointment time. *I agreeClient understands and agrees that Client is 100% responsible and liable for the animal and the animal’s actions at all times, and the Client assumes all risks related to the animal. Client represents and warrants that Client is the owner of the animal and has full and complete authority to enter into this Agreement and make all training decisions for the animal. Client acknowledges that Rover Rehab has not represented, promised, guaranteed, or warranted that the animal will never bite, that the animal will not be dangerous or vicious in the future, that the animal will not exhibit other behavioral problems, or that the results of the training will last for any particular length of time. If the animal causes property damage, or bites or injures any dog, animal, or person (including but not limited to Rover Rehab, Lyz Knight, and their agents), during or after the term of this Agreement, the Client agrees to pay all resulting losses and damages suffered or incurred, and to defend and indemnify Rover Rehab and Rover Rehab's agents from any resulting claims, demands, lawsuits, losses, costs, or expenses, including attorney fees. If the pet is injured in a fight or any other manner during or after the term of the Agreement, the Client assumes the risk and agrees that Rover Rehab and/or it's employees should not be held responsible for any resulting injuries, losses, damages, costs, or expenses. *I agreeI am a force-free, positive reinforcement trainer! This means that I do not advocate for the use of punishment in training and do not use tools like prong collars, choke chains, or shock collars. If you are currently using any of these items and are interested in shifting to positive training I will happily support you as you make that shift, without judgement.
